School  Health Screening Module for Follow up Intervention

Part one  to consist of school/class/class teacher/student- dob/age on …/academic grades overall-subject wise-class rank/participation in ECA including games- conduct with critical observation—smart/not so smart/unsmart-disciplined/undisciplined/appointments if held- weight and height- if aas per standard scale-attendance/regular/irregular- any info of sickness if long absence-health status as per school record-medical history

Part 2 to consist of physical/emotional/intellectual/spiritual quotient on ten point scale as assessed by the class teacher /by warden if a boarder

Part 3 for feedback from the parent for conduct at home generally if a day scholar or occasionally if a boarder/medical history

Part 4 personality profile summary based on MAP test

Part 5 Follow up plan of action as recommended by the Omni board with respect to six dimensions of holistic health

Part 6 Indicative Signs and symptoms as observed by the student and ot teacher and or warden and or parent

Fatigue/Lump or area of thickening that can be felt under the skin/Weight changes, including unintended loss or gain/Skin changes, such as yellowing, darkening or redness of the skin, sores that won’t heal, or changes to existing moles/Changes in bowel or bladder habits/Persistent cough/Difficulty swallowing/Hoarseness/Persistent indigestion or discomfort after eating/Persistent, unexplained muscle or joint pain/Persistent, unexplained fevers or night sweats/Dizziness or lightheadedness/Fainting //Lack of concentration/Blurred vision/Nausea/persistent Cold,  Rapid, shallow breathing//Depression/Thirst// frequent urination/Increased hunger/slow-healing sores or frequent infections/

Part 7 Further testing etc for total diagnosis

Part 8 Therapeutic and or referral recommendation including education and training
